After your initial visit consultation, if eligible, your provider will send brand-name prescription Zepbound, Foundayo or Wegovy to either LillyDirect® or NovoCare®. You will receive a manufacturer email with registration and prescription payment followed by medication delivery to your Alabama residence. No insurance. No prior authorizations.

Care is managed by Alabama-licensed physicians, CRNPs, and Alabama staff. No out-of-state call center.

Monthly cash-pay membership

$175 per month. Visits at month 1 and month 2, then every 3 months. Education and support continue between visits. Optional extra provider visits, plus dietitian and pharmacist add-ons, are available by inquiry. We do not bill insurance.

Cash-pay prescription

If medication is appropriate, we send the prescription to LillyDirect (Eli Lilly) or NovoCare (Novo Nordisk). You’ll receive a registration email, complete cash-pay checkout, and the medication is mailed to your home.

How it works

Most people finish screening in under 5 minutes. Portal documents and labs come next, before the first visit.

    Check eligibility

    Answer a short series of questions. You must meet every criterion to continue. This screens for program fit. It is not a medical visit and does not guarantee a prescription.

    Complete onboarding

    After you pass screening, enter your first name, last initial, and email. You will receive an email with a secure link to create your patient portal account.

    • Log in and complete the pre-visit checklist. Upload:
      • Photo ID
      • Proof of Alabama residency
      • Copy of labs from the last 90 days if performed. If not, you may download the SVH lab order in the portal and take it to your local lab of choice.
    • Complete remaining intake forms.
    • Pay the $175 monthly membership and schedule your first visit.
    • Passing screening and paying for the first month does not guarantee that you will be accepted as a patient or that a medication will be prescribed. The first visit is a paid clinical evaluation.

  3. 3

    Start care

    If medication is appropriate, the prescription is sent to the manufacturer. You purchase Wegovy, Zepbound, or Foundayo from them and it ships to your home.

    Your care team stays with you — including optional dietitian and pharmacist support by inquiry.
